Fry Laboratories is not contracted with any insurance companies. Therefore, we do not complete insurance paperwork for our patients. We apologize for any inconvenience. However, our patients are mailed a receipt along with some Tips and Suggestions for submitting claims to insurance companies on their own.

Payment for Testing
Our patients pay up-front and in full for their tests. Prepayment is necessary and must accompany the requisition form in order for testing to be completed. We accept Visa, Mastercard, or Check only. There will be a $25.00 fee for returned checks.

*** International Patients/Clients: We accept Credit Card only.

MEDICARE Patients
ALL MEDICARE PATIENTS MUST SUBMIT THE FOLLOWING WITH THEIR SPECIMEN:

**We will be unable to perform any tests that are sent without the above materials.**

ABNs must be signed before services are rendered. Given that the blood is only good for 72 hours before testing can begin, we ask that you please send the ABN with your sample.

If your physician has ordered tests from our lab before, their office should have copies of these forms as well. Please be sure to ask for them when getting your blood drawn as they need to be sent in with your specimen.

The ABN (Advance Beneficiary Notice of Noncoverage): Some of Fry Lab’s tests are classified as research-only and are non-FDA approved. Medicare will most likely NOT reimburse for these types of tests. This means that you will be financially responsible for the balance once Medicare has been billed. Please be sure to include a credit card on your requisition before sending in your specimen.

Invoices/Receipts Receipts are mailed out to our patients after processing. If you do not receive your receipt within several weeks of processing, please contact us at (480)-292-8457 to make a request.

Cancellation Policy The ordering physician is the only one allowed to request, change, or cancel tests. If you are thinking about canceling a test for any reason, your physician must submit the cancellation/change in writing to Fry Laboratories. We begin testing the specimen within several days of when we received it. Once testing has begun on the specimen, we cannot cancel the test and payment for services will be required.
